Description of Services:
The Health Records Department maintains, reviews and updates the medical records on all Active Duty, family members and retirees assigned to Boone clinic. There are 3 windows for service in the Health Records Department: active duty, family members and retires, and new record issue. Health records are delivered to the clinic for your appointment; we follow a CLOSED record policy. If you are a walk in appointment please come and we will pull your record for you and take it to your clinic. Allow 24 hours for processing General Medical Power of Attorney.
Allow 15 Business days for processing requests for copying of medical records. .

Patient Contact Representative

Hours of Operation:

0700-1600 Monday - Friday

Description of Services: Serves as special assistant to the Officer in Charge/department heads in matters pertaining to the Patient and Guest Relations Program. The Patient Contact Representative is available to ensure that patient care is delivered in a professional and courteous manner, respecting the dignity and rights of our patients and staff.

Provides solutions and bridge gaps, eases communication, guides people through bureaucratic mazes, acts as liaison and interpreter and works to ensure everyone in the system focused on the patient. The Patient Relations program allows a structured approach to resolving concerns, forwarding suggestions and routing concerns.
